Black Blood is the upgraded form of Burning Blood, doubling end-of-combat HP heal from 6 to 12. You obtain it by swapping Burning Blood during the Act 1 boss-relic pick.
Exceptions: if another boss relic is exceptionally strong (such as an energy relic), giving up Burning Blood for that pick can be correct. Some low-HP builds (Red Skull etc.) find the extra healing excessive, but for between-fight stability the upgrade is usually better.
